Meal Support Referral Form

About the MIND Meal Program

MIND provides free, nourishing homemade meals to individual households in our community who are working through a difficult time — whether that's recovery from illness or surgery, a health challenge, a hard stretch financially, or another life circumstance that's made meals harder to manage right now.

We work hand-in-hand with our Community Partners — local frontline organizations whose case managers, nurses, social workers, and outreach staff know their clients best. Together, we make sure meals reach the people who need them, when they need them.

A few things to know before your apply

Our meals are meant to support your wellbeing, health, healing, and recovery during a specific period of need. We're not a long-term food solution — we're one piece of the care wrapped around you by your Community Partner and other supports in your life.

We have a limited supply of meals each week. We do our best to accommodate every applicant, and we'll let you know about meal availability once your application has been reviewed. If we can't take you on right away, our team or  your referring partner can help connect you to other food resources in the meantime.

Meals are always free. There's no cost, and your information stays private.

A Note to MIND Program Community Partners
If you're a case manager, nurse, social worker, outreach worker, or volunteer making a referral on behalf of a client, thank you. You're the bridge that makes this work. Please fill out Section 1 of the form with your client, or on their behalf if that's easier — we'll follow up once the application is processed.
A Message For Applicants
If someone from one of our  Community Partners has referred you to MIND, you're in the right place. Take your time with the form, skip anything you'd rather not answer, and reach out to your case manager or our team if you need a hand filling it out.
